• Kids Club registration for all families will begin on Wednesday, June 1, 2021 at 8:00 AM. If you plan on utilizing Kids Club at any time in September 2022, you must register by August 1, 2022.

    Kids Club/Mid Club 22/23 Registration

     

    In order to be fully registered you must do each of the following items:

     

    -Fill out the Google form (linked above)

    -Fill out all five registration forms and submit them to raptoua@gpschools.org

    -Once your registration has been processed, you will receive a code to create a brightwheel account. YOU WILL NOT RECEIVE A CODE TO MAKE PAYMENT UNTIL YOUR FORMS HAVE BEEN PROCESSED. 

     

    You cannot attend until all of the above steps are completed.

  • In order to register for Kids Club/Mid Club, all delinquent invoices within the Child Care Department must be paid in full.

     

    This includes: Tuition Based Preschool Programs, Camp O'Fun and Kids Club. 

     

    To begin the registration process, please follow these requirements and complete all forms on the checklist.

    Each school year, parents must re-enroll their child(ren) into Kids Club/Mid Club. Enrollees must email all of the forms linked below. Please fill forms out (in their entirety), print, sign, and scan to: raptoua@gpschools.org. All forms must be filled out and turned in, even if you are a returning family. If you do not have the ability to scan/email documents, please contact Anna Raptoulis at Raptoua@gpschools.org.  The drop-box at Barnes Early Childhood Center will be unavailable during the summer. 

     

    Please allow 3-5 business days for registration processing time. 

    You are not able to begin care the same day you turn in your enrollment forms.

     

    Forms Checklist - all forms must be completed:
     
    1. Enrollment Form and Health Statement (Interactive Form: type info, print, sign)
    • Please fill out the registration information completely. Send completed and signed copies. You must register for a minimum of 3 days. These days must be consistent for the entire month. Should your days change for the following month, contact the Kids Club/Mid Club office immediately. 
    • The Health Statement section at the bottom of the form is required by the state and must be completed and signed.
    • Student ID is not needed.
    • E-Signatures are accepted. 
    2. Child Information Record (Interactive Form: type info, print, sign, email)
    • This form is issued by the state and must be filled out completely. The address cannot read "8 Mile and Mack". The complete address must read: "12345 Mack, Grosse Pointe Woods, MI 48236". Zipcodes must be included. Phone numbers must be filled in. Physician information must be completed or write "none".
    • If you do not have your child's Student ID number or their teacher's information, leave it blank. 
    • E-Signatures are accepted. 
    3. Statement of Rules (please print, sign, email)
    • Return, signed by both parent and child, with registration papers.
    • These rules must be reviewed with each child registering for Kids Club/Mid Club.
    • E-Signatures are accepted.

    4. School-Age Child Care Contract (Interactive Form: type info, print, sign, email)

    • E-Signatures are accepted.

    5. Parent Notification of the Licensing Notebook (print, sign, email)
