The Application for
Conference or Workshop Attendance form is to be used for workshops offered
by groups, organizations or schools held during normal school hours either
in the district or out of the district. No salary credit
will be given for participation. All such activities require
prior approval at the building level and the Substitute Teacher
Coordinator must be contacted.

- Who uses this form?
Any staff member who wants to attend a workshop during the school day on
any teacher duty day is required to use this form.
- How do I do this?
Fill out the form (typically goldenrod in color) and submit it to your
principal or supervisor for approval.
- Where do I send it?
Your building secretary will forward the form to Human Resources after
you receive approval.
- Then what?
You must contact the Substitute Teacher Coordinator in the Human
Resources Department to arrange for your substitute, if one is required.
This applies to all workshop/conference absences, even if they are part
of
a district activity. Call the substitute hot line number to leave

- What if no subs are available?
Sometimes it happens. Permission may be revoked. Your building principal
will be notified and (s)he
will inform you.

How do I get New Teacher Credit?
When you return from the workshop, you must submit a copy of the
application from with verification of attendance and a brief report to
the Office of Staff Development.
